Important Registration Information: New Students registering to Ellis for the 2010-2011 school year must have an Official Louisiana State Immunization Document. It can be obtained from either your child's doctor or a health unit.
You must also provide the following information when registering your child:
withdrawal papers from the other school (if applicable)
Birth Certificate
Official Louisiana State Immunization Document
Social Security Card
Lease from residence or homeowners insurance
Electric bill
Louisiana Driver's License (the address must match that of your electric bill and lease)
